What evidence do I need?
In order for you to fully register with the Disability Advisory and Support Service, you’ll need to complete our registration form, as well as providing us with appropriate evidence of your disability.
This evidence should then be returned to the Disability Advisory and Support Service, along with the registration form, either by email, post, or in person.
What evidence of my mental health difficulty, physical, medical or sensory condition do you need?

You will need to take this guidance sheet to your doctor and ask them to write you a letter that contains the requested information.
The letter must be written in or officially translated to English.
Please ensure that the letter talks about the impact of your condition.
Letters from counsellors or therapists are not accepted. Please ask a doctor to write this letter.
What evidence of my specific learning difficulty, such as dyslexia or dyspraxia, do I need?
- Do you have a report?
If you have an existing report diagnosing dyslexia or dyspraxia, please send this in to us. We can then see whether this can be accepted for use at University, or whether you will need a new report.
If you do need a new report, current students are able to book onto our Do I have dyslexia? workshops, as the first step. Alternatively, it is possible to be assessed outside of the University (see below for further information about this).
- Are you planning on getting a report outside of the University?
If you’re planning on being assessed for dyslexia outside of the University, it’s essential that you make sure that the person completing the report will follow the 2005 SpLD working group guidelines. If they don’t follow these guidelines, you may not be able to access support.
- Are you a current student without a report?
If you don't currently have an assessment report and would like to explore a diagnosis through the University, you’ll need to attend our Do I have dyslexia? workshop (this service is only available for currently registered students). Please book onto the workshop as soon as possible, as we have a limited number of assessment appointments each year.
